One of the best such implementations I've seen
it's the Opera Show Generator -- where you do exactly that:
you fill a form (add/subtract fields, move them around, etc.).
All client-based.
Its result, btw, after "submit" is a HTML/CSS/Javascript-based presentation
that can be run in Opera (and, I think, recent Mozilla implementations).
And of course it can be just shown on the Web.
Saved a lot of our non-techies from Powerpoint bloat, actually.

ps. mind you, I've never actually used the generator with Internet Explorer,
so this particular implementation may not be the end-all solution...
